The wheel didn't fall off, the axle came apart. Look when the wheel comes bouncing back into frame at the end. At least to me it looks like there is an axle hanging off the back of the wheel.

Appetite: lost. Every time I see that video, it makes me sick to my stomach to think how he felt. Especially, when it was a somewhat honest mistake that had to have done a significant amount of damage. Make sure you use the correct lug nuts.
